Patient Medication and Prescription Refill Guide

You may be given written prescriptions to take to your pharmacy, or to mail, if you use a mail-order pharmacy. Please do this at your earliest convenience so they do not get misplaced. The pharmacy will keep them on file until you need them.

Please remember to bring an accurate list and dosages of the medications you are taking every time you come to visit your doctor. Fill out this form for easy reference.

Clinic Policies

Seventy-two (72) hours are needed to complete the approval and refill process.

We now have electronic Prescribing. Please call your pharmacist first if you are in need of a refill.

If your doctor is out of the clinic, and will not return within 72 hours, your request may be forwarded to a different provider for review.

Narcotics and other controlled substance medications can only be authorized by your primary provider. Please call 651-653-0062 to leave a note for your doctor if you are in need of a refill.

Need a Refill?

First contact your pharmacy directly. Please, do not wait until you are totally out of a medication before calling for a refill. Seventy-two (72) hours are needed to complete the approval and refill process.

It is a great idea to call your pharmacy at least one week before your medication runs out.

Medications cannot be refilled by AALFA Providers on Saturdays and Sundays. If the bottle says "0 refills" please call your pharmacy. They will fax us a request for additional refills at 651-653-0288.​You will be notified if your Provider requires you to be seen before a refill is authorized.

Out of Medication?

Do not stop the medication unless you have been told to do so by your doctor.

Call your pharmacy for a refill even if it says “0” refills. The pharmacy will contact us for an OK to refill. Your pharmacy can fax requests to us at 651-653-0288.​You may be required to set up an appointment before your refills can be authorized. Call our office to schedule: 651-653-0062.

Changing Pharmacies?

Call our office Prescription Line (call 651-653-0062, ask for the Prescription Line). Be prepared to leave a message with:

  • New pharmacy name,

  • Telephone number and/or Fax number of the new pharmacy,

  • List of medications and dosages

You may need to be seen for an appointment for any refills, depending on the medication.

Need to change the dosage or quantity of your medication?

Call our office Prescription Line (call 651-653-0062, ask for the Prescription Line). Be prepared to leave a message with:

  • Name of current medication

  • Current dose and frequency

  • New dose and/or frequency you wish to try

Your doctor will review your request and respond within seventy-two (72) hours.

You may need to be seen for an appointment for any medication changes, depending on the medication.

Use a Mail Order Pharmacy?

  • AALFA Family Clinic does not send prescriptions to mail order pharmacies.

  • The patient is responsible for mailing their prescriptions according to their mail order pharmacy's instructions.

  • Make sure to give yourself enough time to get your medications before you run out.

  • Make your yearly appointments in a timely fashion so there is no delay in getting your prescriptions mailed in and refilled. This will help ensure you do not miss any doses of your medication.

Canadian Drug Programs

AALFA Family Clinic is not able to sign or complete any Canadian drug program forms. We can give you a written prescription that you can mail or have filled wherever you wish.
